• This questionnaire is strictly confidential and will be used by the IWF Masters medical team in case of injury/illness during the competition.
  
  It will NOT be used to qualify or disqualify a lifter in terms of their health status or be used by anti-doping.  
  
  All lifters are recommended to see their own personal physician to address their health status prior to engaging in a world Masters competition.
• I declare that I am physically fit to undertake the sport of weightlifting and have no knowledge of any medical condition which will make weightlifting contra-indicated to my well-being.I understand that Travel insurance with health and accident cover is mandatory and also covers me for the activity of weightliftingI am fully aware that all IMWA and EMWA championships are drug tested and I agree to the relevant anti-doping policy.In registering for this championship I agree to follow all the roles and policies on anti-doping and to be tested if selected for doping control. I hereby declare the followings:
  
  1) I acknowledge that I am bound by, and confirm that I shall comply with, all provisions of the International Masters Weightlifting Association (IMWA) Anti-Doping Policy (“IMWA Anti-Doping Policy”), as well as the World Anti-Doping Code (the “WADA Code”) and all International Standards, in particular, the Prohibited List issued by the World Anti-Doping Agency (“WADA”), as amended from time to time, and published on WADA’s website.
  
  2) I acknowledge the authority of the Hungarian Anti-Doping Group Not-for-profit Public Benefit Ltd (“HUNADO”) as a sample collection authority (SCA), and results management authority (RMA) to collect samples, to enforce, to manage results under and to impose sanctions as provided in the applicable anti-doping rules.
  
  3) I acknowledge and agree that any dispute arising out of a decision made pursuant to IMWA Anti-Doping Policy, may be appealed to the appeal bodies laid down in the IMWA Anti-Doping Policy.
  
  4) I acknowledge and agree that the decisions of the arbitral appellate body referenced above shall be final and enforceable, and that I will not bring any claim, arbitration, lawsuit or litigation in any other court or tribunal.
  
  5) I declare and agree that if my doping control results in a positive result, or I commit an anti-doping rule violation (“ADRV”), I am obliged to bear the overall costs of the Doping Control as declared in IMWA Anti-Doping Policy, including the results management, and to pay it within the deadline set by the sanctioning decision. I bear all the additional cost If a Retro-Active Therapeutic Use Exemption application is needed .
  
  6) I have read and understand the present declaration.
